Should I just let them?

My cat is 13yrs old and just received a diagnosis of diabetes. The treatment is very involved and can be expensive. It is not curable and requires constant supervision. Does anyone else out there had to deal with this situation? Is he became ill and suffering? At his age, it is best just to him? HELP!
